AMENDMENT TO HS-P-0033-26
ISSUED TO WAREHOUSE DIRECT
TO PROVIDE HAND SOAP AND PERSONAL CARE ITEMS
FOR THE DUPAGE CARE CENTER
(ONE-TIME PRICE INCREASE OF SPECIFIC ITEMS)
resolution
WHEREAS, on September 8, 2026 through Resolution HS-P-0033-26, the County Board approved a renewed contract to provide hand soap and personal care items, for the DuPage Care Center (hereinafter the “CONTRACT”) between the County of DuPage (hereinafter the “COUNTY”) and Warehouse Direct (hereinafter the “CONTRACTOR”)
WHEREAS, the current CONTRACT, by and through the DuPage Care Center is $44,000.00; and
WHEREAS, after consultation with CONTRACTOR, the COUNTY and CONTRACTOR seek to apply a one-time price increase of specific items in Exhibit A; and
WHEREAS, the Human Services Committee recommends approving the one-time price increase of specific items of the CONTRACT; and
WHEREAS, all provisions of the CONTRACT not expressly changed in the Amendment shall remain the same in their entirety.
NOW THEREFORE BE IT RESOLVED, that the County Board adopts and approved this Amendment to HS-P-0033-26, issued to Warehouse Direct, to apply a one-time price increase of specific items; and
BE IT FURTHER RESOLVED, that one (1) original copy of this Amendment be transmitted to Warehouse Direct at 2001 South Mt. Prospect, Des Plaines, Illinois 60018, by and through the DuPage Care Center.
