TO PROVIDE FLUID DAIRY
FOR THE DUPAGE CARE CENTER RESIDENTS, CAFETERIA AND CAFES’ ON COUNTY CAMPUS
(CONTRACT TOTAL AMOUNT $45,000.00)

WHEREAS, bids have been taken and evaluated in accordance with County Board policy; and
WHEREAS, the Human Services Committee recommends County Board approval for the issuance of a contract to Prairie Farms Dairy, Inc., to provide fluid dairy, for the period of September 25, 2026 through September 24, 2027, for the DuPage Care Center.
NOW, THEREFORE BE IT RESOLVED, that said contract is to provide fluid diary, for the period of September 25, 2026 through September 24, 2027 for the DuPage Care Center per under bid renewal 24-100-DCC, second of three optional renewals, be, and it is hereby approved for the issuance of a contract purchase order by the Procurement Division to Prairie Farms Dairy, Inc., 13938 Keeler Avenue, Crestwood, Illinois 60418, for a contract total amount of $45,000.00.
