WHEREAS, the DuPage County Board has heretofore awarded a purchase order on September 5, 2023, to Lavi Industries, Inc., to provide a streamlined queuing and appointment scheduling system for the Department of Community Services, for the period of September 5, 2023 through September 5, 2026, per GSA Contract #47Q5MA22D08P4; and

 

WHEREAS, the Human Services Committee recommends changes as stated in the Change Order Notice to increase Contract 6593-0001 SERV encumbrance in the amount of $4,015, to the original contract amount of $28,423.18 issued to Lavi Industries, Inc.

 

NOW, THEREFORE BE IT RESOLVED that the DuPage County Board adopts Change Order Notice, dated June 9, 2026, to contract 6593-0001 SERV, issued to Lavi Industries, Inc., to provide a streamlined queuing and appointment scheduling system, for the Department of Community Services, to increase the encumbrance in the amount of $4,015, taking the original contract amount of $28,423.18 and resulting in an amended contract total amount not to exceed $32,758.13.
