WHEREAS, bids have been taken and evaluated in accordance with County Board policy; and
WHEREAS, the Finance Committee recommends County Board approval for the issuance of a contract to Physicians’ Record Company, for printed carbonless forms, for the period of December 1, 2026 through November 30, 2027, for various County Departments.
NOW, THEREFORE BE IT RESOLVED, that said contract is for printed carbonless forms, for the period of December 1, 2026 through November 30, 2027, for various County Departments, per renewal of bid #25-110-FIN, be, and it is hereby approved for the issuance of a contract purchase order by the Procurement Division to Physicians’ Record Company, 1958 Ohio St., Lisle, IL 60532, for a contract total amount of $46,500.00.
