ISSUED TO MESIROW INSURANCE SERVICES, INC.

WHEREAS, County Contract 6736-0001 SERV was issued by the Procurement Division on December 1, 2023; and
WHEREAS, the Finance Committee recommends changes as stated in the Change Order Notice to County Contract 6736-0001 SERV, issued to Mesirow Insurance Services, Inc., to provide Workers Compensation insurance, to increase the contract by $12,886 to pay for the adjusted Workers Compensation premium, resulting in an amended contract total of $212,637, an increase of 6.45%.
NOW, THEREFORE BE IT RESOLVED, that the County Board adopt the Change Order Notice to County Contract 6736-0001 SERV, issued to Mesirow Insurance Services, Inc., to provide Workers Compensation insurance, to increase the contract by $12,886 to pay for the adjusted Workers Compensation premium, resulting in an amended contract total of $212,637, an increase of 6.45%.
