LAKESHORE DAIRY, INC.

FOR FLUID DAIRY

FOR DUPAGE CARE CENTER RESIDENTS, CAFETERIA AND CAFES LOCATED IN THE JTK ADMINISTRATION BUILDING AND JUDICIAL OFFICE FACILITY

(CONTRACT TOTAL AMOUNT $83,000.00)

                     WHEREAS, bids have been taken and evaluated in accordance with County Board policy; and

WHEREAS, the Human Services Committee recommends County Board approval for the issuance of a contract to Lakeshore Dairy, Inc., for fluid dairy, for the period of September 1, 2023 through August 31, 2024, for the DuPage Care Center.

NOW, THEREFORE BE IT RESOLVED, that said contract is for fluid diary, for the period of September 1, 2023 through August 31, 2024 for the DuPage Care Center per bid renewal #22-062-DCC, be, and it is hereby approved for the issuance of a contract purchase order by the Procurement Division to Lakeshore Dairy, Inc, 8424 W. 47th Street, Lyons, Illinois 60543, for a contract total amount of $83,000.00.
